  Drove all the way down to this clinic and was told that the doctor could not see me because I did not fill out the necessary paperwork ahead of time. I had made an appointment 2 weeks prior and was NEVER TOLD that I needed to fill out new patient paperwork, nor did I receive a reminder call. After taking an hour off of work, I will NEVER RETURN TO THIS PLACE!
